Your Gynecomastia Guide

What is Gynecomastia?

Gynecomastia is a condition where boys or men develop enlarged breast tissue. This happens when there is an imbalance in hormones — the body has more estrogen (female hormone) compared to testosterone (male hormone). It can affect one or both sides of the chest and is often seen during puberty, in older age, or because of certain medicines, drugs, or health problems like liver disease.

In some cases, the swelling goes away on its own or with changes in lifestyle or medications. But if the problem doesn’t go away or is more severe, surgery is usually the best and most lasting treatment. The surgery, called gynecomastia surgery (male breast reduction), removes the extra tissue and sometimes fat to give the chest a flatter, more masculine look.

Other treatments usually don’t work well for true gynecomastia, so surgery is often the only effective option. It is generally safe and gives good results with little scarring.

At Kalosa Aesthetics, Dr Ashish Khare is an expert in male breast reduction surgery in Gurgaon – Delhi. With over 20 years of experience, he is one of the most trusted gynecomastia surgeons, having successfully done more than 8,000 surgeries. Kalosa Clinic uses the latest, USFDA-approved technologies like VASER and Microcannula for gynecomastia treatment, ensuring safe and great results.

According to the ISAPS Global Statistics 2020, liposuction is one of the most popular cosmetic surgeries worldwide.

  • A total of 210,737 gynecomastia surgeries have been performed globally.
  • In India, 22,464 male breast surgeries (also known as “man boobs” surgery) have been done.

When Does Gynecomastia Need Surgery?

Gynecomastia surgery is recommended when:

– The breast tissue has been present for more than 1 year and shows no signs of reducing on its own
– Hormonal blood tests are normal, ruling out an underlying medical cause
– Lifestyle changes, weight loss, and medication adjustments have not resolved the condition
– The enlargement causes physical discomfort or emotional distress

Non-surgical treatments such as medications and exercise rarely eliminate true gynecomastia involving glandular tissue. Surgery is the only proven, permanent solution.

Is Gynecomastia Common in India?

According to ISAPS Global Statistics, over 22,464 male breast reduction surgeries are performed in India annually, making it one of the most sought-after cosmetic procedures for men.

Globally, 210,737 gynecomastia surgeries are performed each year.

Grades of Gynecomastia

Causes of Gynecomastia

Gynecomastia develops when there is an imbalance between estrogen and testosterone. The most common causes include:

Hormonal Causes

– Puberty (affects 50–70% of teenage boys; usually resolves within 2 years)
– Ageing (testosterone naturally declines after age 40)
– Hypogonadism (low testosterone production)
– Hyperthyroidism or liver cirrhosis altering hormone metabolism

Medication-Related Causes

– Anabolic steroids and testosterone supplements (bodybuilders — very common in Gurgaon)
– Anti-androgens, anti-ulcer drugs (cimetidine), and some antibiotics
– Heart medications such as digoxin and spironolactone
– Antidepressants including tricyclic antidepressants

Lifestyle Causes

– Obesity — excess body fat increases estrogen production
– Regular alcohol or marijuana use
– Soy-rich diets in excess

Grades of Gynecomastia — Which Grade Are You?

Grade 1 — Mild (Small Enlargement, No Excess Skin)

A small amount of glandular tissue forms behind the nipple, creating a slight puffiness. The chest otherwise looks normal. Most commonly seen in teenagers and young men.
Treatment: VASER liposuction under local anaesthesia. 1-hour procedure, same-day discharge.
Cost at Kalosa: ₹40,000 – ₹50,000

Grade 2 — Moderate (Noticeable Enlargement, No Excess Skin)

The breast tissue is clearly visible through clothing. The nipple-areola complex may begin to protrude. No significant excess skin.
Treatment: VASER liposuction + glandular excision.
Cost at Kalosa: ₹50,000 – ₹65,000

Grade 3 — Significant (Enlargement with Minor Skin Sagging)

Breast tissue is considerably enlarged. Mild skin redundancy begins to appear, especially in the lower chest.
Treatment: VASER liposuction + glandular excision + possible minor skin tightening.
Cost at Kalosa: ₹60,000 – ₹75,000

Grade 4 — Severe (Large Enlargement with Excess Skin)

The breast resembles a female breast in size and shape. Significant skin excess and drooping. Often seen in men with obesity or large weight loss.
Treatment: Full glandular excision + skin resection (may require periareolar or anchor incision).
Cost at Kalosa: ₹70,000 – ₹80,000

About Gynecomastia Surgery

Gynecomastia surgery is a procedure to treat enlarged male breasts, typically caused by hormonal imbalances. It involves removing excess fat and glandular tissue through liposuction or excision. For mild cases, local anaesthesia is used, while more severe cases may require general anaesthesia for safety.

Procedure of Gynecomastia Surgery

Depending on the extent of gynecomastia, surgery can be performed under either general or local anaesthesia. If the gynecomastia is mild, such as in puffy nipples, the procedure can be done with local anaesthesia alone as an outpatient procedure, after which you may go home after about one to two hours of observation. If the gynecomastia is more extensive, then we will use general anaesthesia to ensure safety as well as cosmetic results. Overall, the surgery usually takes about one to two hours, and you can go home the same day. The technology we use is called VASER Liposuction.

As a method of liposuction, VASER uses ultrasound waves to melt the fat, which decreases the effort required to remove the fat. In terms of recovery, VASER liposuction helps decrease bruising, swelling and pain after surgery, allowing for a quicker recovery. VASER liposuction is also an effective method for tightening skin due to collagen remodelling. This technique is designed to ensure that there are no significant signs of surgery, as the incisions you have will be incredibly small (less than 0.5 mm) and placed in an area that are not easily noticed.

Recovery After Gynecomastia Surgery

Gynecomastia Recovery Section
Post-Surgery Guide

Gynecomastia Surgery Recovery — Day-by-Day Guide

Most patients return to work within 2 days and see final results within 2–3 months. Follow this timeline for the smoothest recovery after your gynecomastia surgery at Kalosa Aesthetics, Gurgaon.

Day 0 Pain 3–4/10

Day of Surgery

You go home the same day within 2–4 hours of surgery. A compression garment is applied before you leave the clinic. Take prescribed pain medication as directed. Sleep with your upper body elevated at 30°.

✓ Same-day discharge ✓ Compression garment applied ✓ Elevate upper body 30°
Days 1–3 Pain 2–3/10

Peak Swelling & Rest

Swelling and bruising are at their peak — this is completely normal. Take complete rest. Avoid lifting your arms above shoulder level. Shower is permitted after 48 hours — avoid direct water pressure on incision sites. Pain reduces significantly by Day 3.

✓ Shower after 48 hrs ✓ Complete rest ✗ No arm lifting ✗ No water on incisions
Days 4–7 🏢 Return to Work

Back to Light Activity

Most patients return to desk work and light daily activities. The compression garment must be worn 24 hours a day except during bathing. Sutures (if non-absorbable) are removed on Day 7. You will notice the initial chest shape — though swelling is still present.

✓ Desk work permitted ✓ Sutures removed Day 7 ✓ Garment 24hrs/day
Weeks 2–4 50% swelling gone

Visible Improvement

Swelling reduces by approximately 50%. Light walking and stretching are permitted. Avoid gym, running, or any chest exercise. The chest shape continues to improve as fluid reabsorbs.

✓ Light walking ✓ Stretching OK ✗ No gym ✗ No running ✗ No chest exercise
Weeks 4–6 🚴 Lower Body Exercise

Lower Body Exercise Resumes

You may resume lower-body exercises (cycling, leg press). Upper-body exercise is still restricted. Compression garment may be worn only during the day from Week 4 — no longer required at night.

✓ Cycling & leg press ✓ Daytime garment only ✗ Upper body still restricted
Week 6+ 💪 Full Gym Cleared

Full Exercise Cleared

Full exercise including chest workouts is now permitted. The compression garment is no longer needed. Results are approximately 70–80% visible at this stage — chest shape is clearly transformed.

✓ Full gym including chest ✓ No compression garment ✓ 70–80% results visible
3 Months ⭐ Final Results

Final Results Visible

Final results are visible once all residual swelling subsides and skin settles into its new position. Scarring from the micro-incisions is typically invisible to the naked eye by this point. Your new masculine chest contour is fully revealed.

Risks Involved in Gynecomastia Surgery

Although gynecomastia surgery typically has a high success rate, it is important to realise that risks may occur. The most common risks identified with surgery are hematoma (a collection of blood) and seroma (a collection of body fluid). The following may also be associated with surgery:

Infection: There is a potential risk for infection where the surgery was conducted, which will produce redness, swelling, and pain in the area.

Nipple Sensation Changes: Surgery can cause temporary and/or permanent changes in nipple sensation.

Chest Asymmetry: One side of your chest can often look different from the other, or one side can look more different than the other; this means perfect symmetry on both sides can often not be guaranteed.

Skin Irregularities: The skin in the area where surgery was performed can appear uneven or may have dimpling, which may impact the visual appearance of the surgical site.

Most of these issues are easily treated and are temporary in the appropriate circumstances; however, it is advised that you make clear and discuss all the surgical risks with your surgeon before surgery.

Pre-Care Tips for Gynecomastia Surgery

Consultation & Evaluation

In advance of the operation, have a thorough discussion with your surgeon. You must discuss your medical history and any allergies you may have, as well as any medications you are taking. Ensure that any underlying health conditions are stable.

Stop Taking Certain Medications

It is recommended to stop any medications which affect clotting (e.g. aspirin, ibuprofen, some herbal supplements [vitamin E, fish oil], etc.) 1 to 2 weeks before surgery to lessen the chances of bleeding during your operation. It is best to adhere to your surgeon’s instructions about which medications you’re able to take before surgery.

Avoid Smoking and Alcohol

You should avoid smoking and limit alcohol intake at least 2 weeks before surgery to enable the best postoperative healing and the effects of anaesthesia.

Arrange For Transportation

If you are undergoing an operation which involves anaesthesia, you will need someone to drive you to and from the surgery.

Fasting Before Surgery

Follow your surgeon’s orders regarding fasting. Most surgeons recommend to put aside food and drink for 6 -12 hours before your surgery.

Post-Care Tips for Gynecomastia Surgery

Compression Garment

Wear the compression garment as directed (typically 24/7 for the first few weeks) to help minimise swelling and aid in healing. It is fine to take it off for showering, but no more than a few minutes.

Rest and Recovery

Do not push yourself for the first 72 hours following surgery and get lots of rest. The less you stress your body physically, the faster you heal – so take it easy. No heavy exercise or lifting for at least 4-6 weeks.

Wound Care

Keep the incision areas clean and dry. Follow your surgeon’s directions for cleaning and applying ointments. Keep your hands away from the surgical area. Avoid touching or rubbing the surgical area to reduce your chances of introducing bacteria.

Follow-Up Appointments

Attend all follow-up appointments for your surgeon to assess your healing and address stitches as needed.

Pain Management

Take the prescribed pain medications as indicated, and you can take over-the-counter pain medications as directed.

Monitoring for Potential Complications

Be alert for signs of infection (redness, swelling, fever), hematoma (blood pooling), or seroma (fluid buildup). If you see any of these complications, please notify your surgeon as soon as possible.

Gradual Return to Activity

After 1 week, you can resume light activities such as walking or stretching. Increase your activity slowly and do not return to heavy workouts or lifting for at least 6 weeks.

Diet and Hydration

Be sure to drink plenty of water and eat healthy foods, as this will help speed your recovery.

Nipple Sensation

You may notice some changes in nipple sensation after surgery, but that should return gradually. If you are concerned about any numbness or discomfort lasting longer, let your surgeon know.

Patience With Results

You may be waiting a few months for optimal results as swelling goes down. Be patient and always follow your surgeon’s directives during your recovery.

The cost of gynecomastia surgery depends on the following factors:

  • Doctor’s Fees: The fee charged by the doctor for the consultation and surgery.

  • Severity of Gynecomastia: The level of gynecomastia at the time of surgery (i.e., mild to severe).

  • Treatment Procedure: The surgery treatment option chosen (e.g., liposuction or excision).

  • Anaesthesia: Whether it was local anaesthesia or general anaesthesia.

  • Diagnostic Tests: Any tests that must have been conducted before surgery.

  • Hospitalisation Fees: Hospitalisation fees if one is required.

  • Medications: The cost of any medicines, including those used before and after the surgery.

  • Post-Surgery Care: Follow-up visits and recovery care.

Q: What is the cost of gynecomastia surgery in Gurgaon?

A: Gynecomastia surgery at Kalosa Aesthetics in Gurgaon costs between ₹40,000 and ₹80,000 depending on the grade of gynecomastia and the surgical technique required.

Grade 1 (puffy nipples) starts at ₹40,000 under local anaesthesia. Grade 4 (severe with excess skin) costs up to ₹80,000 under general anaesthesia. EMI options are available.

Q: Is gynecomastia surgery permanent?

A: Yes. Once the glandular breast tissue and excess fat are surgically removed, the results are permanent in the vast majority of cases. The removed tissue does not grow back.

However, new gynecomastia can develop if the underlying hormonal cause is not addressed — for example, continued steroid use or hormonal medication. Maintaining a stable weight post-surgery helps preserve results long-term.

Q: How long does gynecomastia surgery take?

A: The procedure takes 1–2 hours depending on the grade of gynecomastia. Grade 1 cases (liposuction only) are typically completed in 45–60 minutes.

Grade 3 and 4 cases requiring glandular excision and possible skin correction take up to 2 hours.

Q: Is gynecomastia covered by insurance in India?

A: Standard health insurance policies in India classify gynecomastia surgery as cosmetic and do not cover it. In rare cases where gynecomastia is causing physical pain, skin infection, or is confirmed to be caused by a diagnosed hormonal or hepatic condition, partial insurance coverage may be possible.

Dr. Khare provides detailed medical documentation to support insurance claims when applicable. EMI financing is available at Kalosa for all patients.

Q: What is the difference between VASER liposuction and traditional liposuction for gynecomastia?

A: VASER uses focused ultrasound waves to selectively liquefy fat cells before removal, while traditional liposuction uses mechanical force alone. For gynecomastia, VASER is significantly superior: it removes fat more precisely, preserves surrounding nerves and skin, causes 40–60% less bruising, enables faster recovery, and actively promotes skin tightening through collagen stimulation. Kalosa has used USFDA-approved VASER technology since 2012.

Q: Can gynecomastia come back after surgery?

A: Recurrence is rare (less than 5% of cases) when surgery is performed by an experienced surgeon who removes all glandular tissue. The most common cause of recurrence is resuming anabolic steroids or medications that trigger hormonal imbalance. Weight gain after surgery can cause fatty tissue regrowth but does not recreate true glandular gynecomastia.

Q: How do I know if I have glandular gynecomastia or just chest fat (pseudogynecomastia)?

A: The simplest self-test: place your thumb and index finger on either side of the nipple and press inward slowly. If you feel a firm, rubbery disc of tissue underneath the nipple — that is glandular tissue (true gynecomastia). If the chest feels soft throughout with no firm central lump — it is likely pseudogynecomastia (chest fat only). Dr. Khare confirms this with a clinical examination and ultrasound during your consultation.

Q: What is the best age for gynecomastia surgery in India?

A: Ideally, surgery is performed after the age of 18 when hormonal levels have stabilised. Teenage gynecomastia (during puberty) is often observed for 12–24 months before surgery is considered, as it frequently resolves on its own. There is no upper age limit — Dr. Khare has successfully performed gynecomastia surgery on patients in their 50s and 60s.

Q: Is gynecomastia surgery painful?

A: Pain is minimal with VASER liposuction. Most patients describe post-operative discomfort as a 3–4 out of 10 on the pain scale, which is well managed with prescribed oral pain medication for the first 48–72 hours. General anaesthesia cases feel nothing during surgery. By Day 3, the majority of patients are comfortable and mobile without pain medication.

Q: Will there be visible scars after gynecomastia surgery?

A: With VASER liposuction, incisions are smaller than 0.5 mm — essentially invisible after 3–6 months of healing. For cases requiring glandular excision, a small periareolar incision is made along the lower edge of the areola, which heals along the natural colour boundary and becomes barely visible. Grade 4 cases may require slightly longer incisions, which Dr. Khare places in natural skin folds for maximum concealment.
